New Patient Appointment Information
All new pulmonary patients should plan on their visit taking 2 hours. We will do a comprehensive examination and perform diagnostic tests in order to properly evaluate your pulmonary health. If you have recently (within 30 days) had a chest x-ray, pulmonary function test, or other diagnostic testing performed, be sure to mention that to our staff when making an appointment.


You will also need to bring your insurance card, drivers license, co-pay (when applicable), and the following forms to your appointment:


New Patient Information

Financial Policy
HIPAA Authorization to Speak with Family

What to Expect (New Patients)
Whenyou arrive, you will go to the front desk and sign in, letting the front desk personnel know you have arrived. You will then submit your paperwork to them. They will check your forms to make sure they are complete; scan your insurance card; and collect your co-pay. They will then notify the medical assistant who will perform your PFT that you have arrived. You will then complete the one hour PFT. After your PFT, you will have your chest x-ray taken. (Please notify the staff prior to your chest x-ray if you think you may be pregnant). Once the chest x-ray is developed, you will meet your new physician who will give you a physical exam and review your medical history and all diagnostic tests.

Sleep Patient Information (New and Established)
When you arrive, you will go to the front desk and sign in, letting the front desk personnel know you have arrived. You will then submit your paperwork to them. They will check your forms to make sure they are complete; scan your insurance card and your driver’s license; and collect your co-pay. They will then notify the medical assistant who will review your medical information, take vital signs and review your medications.  After the medical assistant has completed your work-up, then you will be moved to the physician’s exam room where you will meet with your physician. 

New Sleep Patients will need to bring the following items to their appointment:
